Community Access Occupational Therapist – Adults, Phys Med & Geriatric Health (Scarborough Region)
Monday to Friday, 8-hour shift

VHA’s Community Access Occupational Therapist is part of an innovative model of client care contributing to the rapid increase in home care services for the aging population. This is a key team member to ensure Ontario Health atHome waitlisted clients have access to OT services in a timely manner. This role also provides administrative and scheduling support so you can focus on direct client care. #LI-VHA
Key Areas of Accountability
VHA’s Community Access Occupational Therapists have the following key areas of accountability, but not limited to:
Assesses and implements Occupational Therapy Interventions in a community setting in accordance with professional practice guidelines and standards established by VHA and the perspective regulatory college.
Assesses clients using interviews, standardized and informal assessment techniques in accordance with professional practice guidelines and standards established by VHA Rehab Solutions.
Interventions include but are not limited to: Home safety equipment and mobility aids, falls preventions and energy conservation strategies, palliative care, ADL Training, cognitive remediation/compensatory strategies, and linking to community resources/funding.
Attend client related meetings to report on assessment findings and discuss plans for interventions, progress reports and discharge as required.
Ensure coordinated delivery of care through ongoing communication with all members of the client’s team regarding plans and schedules for interventions.
Document all assessment findings, treatment plans, progress and discharge summaries of interventions as required by the profession, referral source and the agency.
Ensure that services provided are best practice, evidence based and evaluated through the routine use of valid and reliable outcome measurement.
Adheres to the code of ethics of the profession, in maintaining standards of practice for the profession as defined by the College of Occupational Therapist of Ontario.
Required Qualifications:
Degree in Occupational Therapy from a recognized school of Occupational Therapy.
Member in good standing with the College of Occupational Therapists of Ontario.
Membership in professional association preferred.
Experience within the professional discipline is an asset.
Specialized courses to support areas of clinical expertise.
Ability to work independently.
Familiar with the routine use of valid framework and reliable outcome measures to support clinical interventions.
Demonstrated commitment to client and family centered care.
Demonstrated ability to recognize safety concerns, communicate effectively using a computer and/or mobile device
